The Homelessness Prevention (HP) Case Manager will provide one-on-one intensive case management to clients who are at imminent risk of homelessness or persons currently homeless. By providing supportive services, the HP Case Manager will determine and verify client needs, provide available financial and in-kind assistance, and arrange for or provide referrals to appropriate outside services including housing resources.

The position of HP Case Manager is an integral role in the Case Management Team. The HP Case Manager will receive direction from the Program Manager of Homelessness Prevention and Rapid Re Housing, and Director of Programs and Services. This position requires you to think on your feet, independently and collaboratively make responsible decisions, and creatively problem solve situations unique to the client's circumstances. This position entails that you can not only work as a vital member of a team, but also be efficient while working independently.

Bilingual Spanish or Mandarin Case Manager

• Provide intensive case management services to low-income and homeless clients who qualify including goal setting, long-term case plan development, progress monitoring, money management, tenant education and referrals.

• Collaborate with clients on outcomes and goals set forth on the Housing Stability Plan, reviewing and evaluating family progress on an ongoing basis and adjusts case plan as appropriate.

• Assess and verify client needs, determining eligibility for a variety of financial and in-kind emergency assistance services.

• Data entry into Clarity and Salesforce databases.

• Maintain thorough and accurate progress notes, files, and correspondences in Clarity and Salesforce databases. Organize and maintain case files in a timely fashion and in accordance with professional practice.

• Connect clients and their families with needed and available community resources.

• Maintain professional rapport with clients, peers, and other community service providers.

• Communicate with clients and agencies to document use/success of outgoing referrals.

• Provide all required documentation in a timely manner, which may include client follow-up, outcome evaluation, client contact sheets and evaluations.

• Identify and secure appropriate rental housing for client, when appropriate.

• When necessary, pre-screen and evaluate all prospective clients for acceptance and eligibility into homelessness prevention programs using judgment based on professional knowledge of requirements set forth in the grant.

• Solid understanding of requirements of grants, contracts and federal regulations.

• As assigned, participate in agency public relations activities such as agency fairs and community presentations of agency programs and services.

• As assigned, represent the agency on boards and committees.


• Education and Experience:

• Minimum of Bachelor's Degree in Social Work or Related Degree.

• Two years' experience in social work and experience working with low-income and at-risk communities. Experience working in a crisis setting and ability to respond appropriately in an emergency.
